On the establishment of forms of election documents for the election 

of members of the Mazhilis of the Parliament and maslikhats of the 

Republic of Kazakhstan

 

In accordance with subparagraph 6) of Article 12 of the Constitutional Act of the Republic of Kazakhstan dated September 28, 1995 on Elections in the Republic of Kazakhstan, the Central Election Commission of the Republic of Kazakhstan RESOLVES:

1. To establish the attached forms of election documents for the election of Mazhilis Members to the Parliament of the Republic of Kazakhstan according to party lists:

1) statement of a citizen about his/her consent to be included in the party list (Annex 1);

2) party list (Annex 2);

3) protocol of the Central Election Commission of the Republic of Kazakhstan on establishing the compliance of candidates for Mazhilis Members to the Parliament of the Republic of Kazakhstan with the requirements of the Constitution of the Republic of Kazakhstan and the Constitutional Act of the Republic of Kazakhstan on Elections in the Republic of Kazakhstan (Annex 3);

4) protocol of the Central Election Commission of the Republic of Kazakhstan on establishing the non-compliance of a candidate for Mazhilis Member to the Parliament of the Republic of Kazakhstan with the requirements of the Constitution of the Republic of Kazakhstan and the Constitutional Act of the Republic of Kazakhstan on Elections in the Republic of Kazakhstan (Annex 4);

5) protocol of the Central Election Commission of the Republic of Kazakhstan on registration of the party list for the election of Mazhilis Members to the Parliament of the Republic of Kazakhstan (Annex 5);

6) certificate of a candidate for the Mazhilis Member to the Parliament of the Republic of Kazakhstan, included in the party list (Annex 6);

7) protocol of the territorial election commission on the registration of proxies of a political party (Annex 7);

8) certificate of a proxy of a political party (Annex 8);

9) protocol of the precinct election commission on the results of voting on the elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected according to party lists, at the polling station (Annex 9);

10) protocol of the district, city, district in the city election commission on the results of the elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected according to party lists (Annex 10);

11) protocol of the the election commission of region, cities of republican significance and the capital on the results of the elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected according to party lists (Annex 11);

12) protocol of the Central Election Commission of the Republic of Kazakhstan on the results of elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected according to party lists (Annex 12);

2. To establish the attached forms of election documents for the election of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in single-seat territorial constituencies:

1) statement of a citizen on consent to run as a candidate for Mazhilis Member to the Parliament of the Republic of Kazakhstan in a single-seat territorial constituency from a political party, other public association (Annex 13);

2) statement of a citizen about his/her intention to run as a candidate for Mazhilis Member to the Parliament of the Republic of Kazakhstan in a single-seat territorial constituency (Annex 14);

3) protocol of the district (territorial) election commission on establishing compliance (non-compliance) of candidates for Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in single-seat territorial constituencies, with the requirements of the Constitution of the Republic of Kazakhstan and the Constitutional Act of the Republic of Kazakhstan on Elections in the Republic of Kazakhstan (Annex 15);

4) protocol of the district (territorial) election commission on the registration of candidates for Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in a single-seat territorial constituency (Annex 16, 16-1);

5) certificate of a candidate for Mazhilis Member to the Parliament of the Republic of Kazakhstan, elected in a single-seat territorial constituency (Annex 17);

6) protocol of the district (territorial) election commission on the registration of candidate’s proxies (Annex 18);

7) certificate of a proxy of the candidate (Annex 19);

8) protocol of the precinct election commission on the results of voting on the elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in a single-seat territorial constituency, in the polling station (Annex 20);

9) protocol of the territorial election commission on the results of voting on the elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in a single-seat territorial constituency (Annex 21);

10) protocol of the district (territorial) election commission on the results of voting on the elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in a single-seat territorial constituency (Annex 22);

11) protocol of the Central Election Commission of the Republic of Kazakhstan on the results of elections of Mazhilis Members to the Parliament of the Republic of Kazakhstan, elected in single-seat territorial constituencies (Annex 23);

3. To establish the attached forms of election documents for the election of members of maslikhats of the Republic of Kazakhstan according to party lists:

1) statement of a citizen on consent to be included in the party list for the election of members of maslikhat of the Republic of Kazakhstan (Annex 24);

2) party list (Annex 25);

3) protocol of the territorial election commission on establishing the compliance of a candidate for member of maslikhat of the Republic of Kazakhstan with the requirements of the Constitution of the Republic of Kazakhstan and the Constitutional Act of the Republic of Kazakhstan on Elections in the Republic of Kazakhstan (Annex 26);

4) protocol of the territorial election commission on establishing the non-compliance of a candidate for member of maslikhat of the Republic of Kazakhstan with the requirements of the Constitution of the Republic of Kazakhstan and the Constitutional Act of the Republic of Kazakhstan on Elections in the Republic of Kazakhstan (Annex 27);

5) protocol of the territorial election commission on registration of the party list of candidates for members of maslikhat of the Republic of Kazakhstan (Annex 28);

6) certificate of a candidate for member of maslikhat of the Republic of Kazakhstan, included in the party list (Annex 29);

7) protocol of the territorial election commission on the registration of proxies of the political party (Annex 30);

8) certificate of a proxy of a political party (Annex 31);

9) protocol of the precinct election commission on the results of voting on the election of members of maslikhat according to party lists (Annex 32);

10) protocol of the territorial election commission on the results of voting on the election of members of maslikhat according to party lists (Annex 33);

11) protocol of the territorial election commission on establishing the results of the elections of members of maslikhat according to party lists (Annex 34);

4. To establish the attached forms of election documents for the election of members of maslikhats of the Republic of Kazakhstan in single-seat territorial constituencies:

1) statement of a citizen about his/her consent to run as a candidate for member of maslikhat in a single-seat territorial constituency from a political party, other public association (Annex 35);

2) statement of a citizen about his/her intention to run as a candidate for member of maslikhat in a single-seat territorial constituency (Annex 36);

3) protocol of the district (territorial) election commission on establishing compliance (non-compliance) of a candidate for member of maslikhat of the Republic of Kazakhstan with the requirements of the Constitution of the Republic of Kazakhstan and the Constitutional Act of the Republic of Kazakhstan on Elections in the Republic of Kazakhstan (Annex 37);

4) protocol of the district (territorial) election commission on the registration of candidates for members of maslikhat of the Republic of Kazakhstan in a single-seat territorial constituency (Annex 38, 38-1);

5) certificate of a candidate for members of maslikhat of the Republic of Kazakhstan in a single-seat territorial constituency (Annex 39);

6) protocol of the district (territorial) election commission on the registration of the candidate’s proxies (Annex 40);

7) certificate of a proxy of the candidate (Annex 41);

8) protocol of the precinct election commission on the results of voting for the election of maslikhat members in a single-seat territorial constituency (Annex 42);

9) protocol of the district (territorial) election commission on the results of voting on the election of members of maslikhat in a single-seat territorial constituency (Annex 43);

10) protocol of the territorial election commission on the establishment of the results of the elections of members of maslikhat (Annex 44);

5. To establish the attached forms of election documents for the election of Mazhilis Members to the Parliament of the Republic of Kazakhstan and maslikhats of the Republic of Kazakhstan:

1) the voter list for the election of Mazhilis Members to the Parliament, elected according to party lists, and maslikhats of the Republic of Kazakhstan (Annex 45);

2) certificate of an observer of a foreign state or an international organisation (Annex 46);

3) decision of the territorial election commission on establishing a different time for the start and end of voting (Annex 47);

4) decision of the territorial election commission on approval of the list of polling stations where voting can be completed at any time (Annex 48);

5) protocol of the precinct election commission on the cancellation of unused absentee voting certificates (Annex 49);

6) protocol of the territorial election commission on the use of absentee voting certificates (Annex 50);

7) protocol of the precinct election commission on the opening of the polling station (Annex 51);

8) notification of the territorial election commission on the number of polling stations and the number of voters included in the lists, as of the day the voter lists are submitted for familiarisation (Annex 52);

9) notification of the territorial election commission on the turnout of voters for the elections of Members of the Mazhilis to the Parliament and maslikhats of the Republic of Kazakhstan (Annex 53).

6. To publish this resolution on the website of the Central Election Commission of the Republic of Kazakhstan.
